Program Details
- Full initial payment due upon joining
- Renewal payment is due on the first day of the anniversary month
- Copay must be paid in full at time of service to qualify for any courtesy savings.
- It is the sole responsibility of the member to maximize benefits by arranging the appropriate appointments within the 12 month period. If the appointments are not used, the member will not be entitled to a refund.
- Membership runs on a 12 month period
- Payments made with: Cash or Check receives 20% Courtesy Savings.
- Payments made with: Credit Cards receives 10% Courtesy Savings.
Exclusions
- Cannot be used in conjunction with any other dental plan or dental insurance
- Cannot be used for services for injuries covered under workman’s compensation claims
- Non Transferable
- Implants reduced by (10% Cash or Check and 5% Credit or Debit Cards)
- Orthodontic Treatment (Invisalign and Clear Correct)
- Discounts cannot be applied to treatment completed before membership
- Any third-party financing and/or Care Credit does not qualify for any courtesy savings.
